Choose an example from the media of an organizational failure in the public sector. Summarize the failure (the summary should be approximately one to two pages) and try to determine if it matches one of O’Hara’s categories of failure. Then, using concepts from O’Hara’s or Rainey’s textbook try to analyze what might have gone wrong organizationally to bring about that failure and then propose management solutions of how such a failure might be prevented in the future.
